BECOME A MEMBER

ETANNI WELLNESS CENTER MEMBERSHIP AGREEMENT

This Membership Agreement (the "Agreement") is made and entered into on the date of acceptance by the

member (the "Member") and **Etanni Wellness Center** ("Etanni"), a business operating in the State of Washington.

By purchasing a membership, the Member agrees to abide by all terms and conditions outlined below.

**1. MEMBERSHIP TERMS**

1.1 **Membership Eligibility:** Membership is available to individuals who are at least 18 years old or have parental/guardian consent if under 18.

1.2 **Membership Benefits:** Membership grants access to Etanni’s facilities, services, and wellness programs in accordance with the selected membership plan. Additional services may be available for an extra fee.

1.3 **Membership Term:** Membership is available on a month-to-month or annual basis, as selected by the Member.

**2. FEES AND PAYMENT**

2.1 **Membership Fees:** The Member agrees to pay the applicable membership fee based on the selected plan. Fees are non-transferable and non-refundable except as provided under Section 4.

2.2 **Billing Cycle:** Membership fees will be billed on a recurring basis per the selected billing cycle (monthly or annually). The Member authorizes Etanni to process these payments automatically.

2.3 **Late Payments:** If payment is not received within seven (7) days of the due date, a late fee of \$25 may be applied.

**3. MEMBER OBLIGATIONS AND FACILITY USE**

3.1 **Code of Conduct:** Members agree to adhere to Etanni’s rules and policies, respect staff and fellow members, and use the facility in a responsible manner.

3.2 **Health and Safety:** The Member acknowledges that participation in wellness activities involves certain risks and voluntarily assumes responsibility for any injuries or damages sustained. Members should consult a physician before engaging in physical activities.

3.3 **Personal Belongings:** Etanni is not responsible for lost, stolen, or damaged personal property.

**4. REFUND POLICY**

4.1 **Three-Day Full Refund:** A Member may cancel their membership for a full refund if cancellation is requested within three (3) calendar days of signing this Agreement. To request a refund, the Member must submit a written notice via email or in person within the 3-day period. Refunds will be processed within 10 business days.

**5. MEMBERSHIP CANCELLATION**

5.1 **Cancellation Process:** Members may cancel their membership at any time by submitting a cancellation request through Etanni’s online portal or in writing.

5.2 **Cancellation Fee:** If a Member cancels their membership, they must pay a cancellation fee equal to one (1) month’s membership cost. This fee will be charged at the time of cancellation.

5.3 **Termination by Etanni:** Etanni reserves the right to terminate this Agreement for violations of policies, non-payment, or conduct deemed inappropriate by management. No refunds will be issued in cases of termination due to violations.

**6. LIABILITY WAIVER**

6.1 The Member understands and agrees that Etanni is not liable for any injuries, health conditions, or damages incurred while using the facility or participating in services. The Member releases and discharges Etanni, its employees, and affiliates from any and all liability arising from participation in activities or use of the premises.

**7. GOVERNING LAW**

7.1 This Agreement shall be governed and interpreted in accordance with the laws of the State of Washington. Any disputes shall be resolved in a court of competent jurisdiction in Washington State.

**8. ACKNOWLEDGMENT AND ACCEPTANCE**

By purchasing a membership, the Member acknowledges that they have read, understood, and agreed to the terms of this Agreement.
